Parents and Guardians of Wautoma Area School District Student Athletes may fill out the online travel release form prior to their child competing. Please note: If a parent/guardian has arranged for another adult (non-student) to take their child home, the individual will need to show identification to the coach(s) prior to the student being released from the event.

ALL ONLINE TRAVEL RELEASE FORMS MUST BE COMPLETED BY 1:00 PM ON THE DAY OF A WEEKDAY EVENT.
If release forms are submitted after 1:00 PM the parent/guardian will have to fill out a paper form with the coach(s) prior to departure.

ALL ONLINE TRAVEL RELEASE FORMS MUST BE COMPLETED BY 3:00 PM ON FRIDAY BEFORE A WEEKEND TOURNAMENT.
If release forms are submitted after 3:00 PM the parent/guardian will have to fill out a paper form with the coach(s) prior to departure.
